Welcome to our outreach clinic located at the Oaklands Health Centre in Holmfirth, where our expert Consultants attend to see you in the community, closer to where you live, to provide you with top-tier care for a healthier, pain-free life.
You can request your GP to refer you on the NHS via the e-Referral Service to the Oaklands Health Centre, in order to book an appointment with one of our specialist Consultants and discuss the treatment options available to you. You can still be referred to the Oaklands Health Centre, even if this is not your GP practice.

Oaklands Health Centre is an outpatient facility only. During your initial appointment, your Consultant will discuss the reasons for your referral and your symptoms. Should you require surgery, One Health’s designated Patient Liaison Department will contact you following your appointment to arrange this at one of our partnering hospital locations. The Oaklands Health Centre Clinic is on Huddersfield Road (A6024), and close to Holme Valley Memorial Hospital and situated next to Elmwood Family Doctors. Holmfirth is a small town just north of the Peak District in West Yorkshire where the Holme and Ribble rivers meet, just 6 miles south of Huddersfield and 13 miles northeast of Glossop.
