Welcome to our outreach clinic located at the Moorfield House Surgery in Leeds, where our expert Consultants attend to see you in the community, closer to where you live, to provide you with top-tier care for a healthier, pain-free life.
You can request your GP to refer you on the NHS via the E-Referral Service to the Moorfield House Surgery, in order to book an appointment with one of our specialist Consultants and discuss the treatment options available to you. You can still be referred to the Moorfield House Surgery, even if this is not your registered GP practice.

Moorfield House Surgery is an outpatient facility only. During your initial appointment, your Consultant will discuss the reasons for your referral and your symptoms. Should you require surgery, One Health’s designated Patient Liaison Department will contact you following your appointment to arrange this at one of our partnering hospital locations.
